
Since the launch of E.ON U.S.'s wellness program in 1995, the company has offered a variety of benefits and incentives to support the health and well-being of you and your families. Not only are you improving your health through these programs, you have said that these activities bring you closer to your family and colleagues.
During the 2010 Open Enrollment, employees will have the option of committing to participate in the expanded ChooseWell incentive program.
To take advantage of the program, employees must complete these steps in 2010 and 2011:
- Attend a WellFair or release requested biometric measures obtained from a physical with your personal health care provider to KC WELLNESS, the company's single-source health fair provider (by April 30, 2010);
- Complete an online Health Risk Assessment.
The ChooseWell incentive payment, as well as the new Choose to Own Your Health incentive, will be applied to your HCRA after an 18-month period so you have sufficient time to commit to a goal and see the results of your wellness efforts. This allows you time to get your results from a health fair or your personal health care provider, as well as your Health Risk Assessment, and follow up on a suggested regimen. It also allows you to compare two years of health fair data to see if your wellness efforts are making a difference in your biometric measures including blood pressure, cholesterol, weight and glucose levels.
E.ON U.S.'s wellness programs are here to help you live longer, healthier and more productive lives. To receive your 2010/2011 ChooseWell incentive, follow these steps:
- In 2010 and 2011, attend a company WellFair OR visit your personal health care provider between January 1 and April 30 both years and release requested biometric measures to KC WELLNESS, the company's single-source health fair provider, and
- Complete an online Health Risk Assessment through one of the following sites:
- Go to www.anthem.com for all employees who are enrolled in Anthem's EPO or PPO plan; or
- Go to www.webmdhealth.com/sponsors/eon.htm for employees who have waived medical benefits through Anthem.
- Receive $200 in your Health Care Reimbursement Account in July 2011. *
